We partnered with the NYU Langone Pediatric Center because while they deliver top-tier medical care, our targeted grant bridges the gap for Hempstead's most underserved families—ensuring an elite healthcare institution can provide true whole-family support.

They handle the medicine; we fund the rest.

Shifting the Medical Model to Whole-Family Care

Local demographics and social challenges require a departure from traditional medicine. By integrating non-clinical support directly into the medical team, NYU Langone pivoted to treat the child’s whole environment.
Council 1869 funds this innovation by embedding Patient Care Associates and Family Support Counselors directly into the care team to dismantle barriers like food insecurity, education access, and transportation—meeting families exactly where they are.

Where The Money Goes

Council 1869 funds the support systems that turn healthcare into whole-family care, including:

Family Support Counselors who provide immediate support and connect families to long-term community resources
Patient Care Associates who help families navigate appointments, paperwork, transportation, and language barriers
Emergency Resource Fund providing essentials like diapers, formula, cribs, clothing, meals, and hygiene supplies for families in crisis

Our Commitment

In early 2024, Council 1869 made a $500,000 multi-year grant commitment to help ensure essential family support services remain a permanent fixture at the Hempstead Center.

This investment strengthens the Center’s ability to serve families consistently, respond to complex needs, and build a more sustainable model of care for children in the community.
